Manage Your Plan
Make changes to your policy by logging in to My Health Plan and selecting “Change Policy.” From February 1 through October 31st a valid qualifying life event is required for most changes.

2023 Tax Form 1095-B
Our members may download their 2023 Tax Form 1095-B from My Health Plan after February 26, 2024. To request a paper copy, please contact Customer Care at the number on the front of your member ID card or at the bottom of this page, email your request to CustomerService@medmutual.com, or submit a written request to P.O. Box 94917, Cleveland, OH 44101-4719.
