Superwell ®Disease and Maternity Management Program

Medical Mutual offers the SuperWell Disease and Maternity Management
Program for our members who have been diagnosed with a chronic disease or who are
currently pregnant to provide additional support and education that complements
what they receive from their physician. When a member is enrolled in the SuperWell
Disease and Maternity Management Program all co-morbid conditions in the list below
are addressed. Health Coaches work with the member to teach them how to avoid potential
complications and the importance of complying with their prescribed treatment plan.
The SuperWell Disease and Maternity Management Program is available
to eligible members with one or more of the following conditions:
- Asthma
- Chronic obstructive pulmonary disease
- Chronic pain conditions
- Congestive heart failure
- Coronary artery disease
- Depression
- Diabetes
- Pregnancy
