Does my plan cover Gastric Bypass services?

Gastric Bypass is an invasive surgery that should only be used after all other avenues are exhausted. Coverage for gastric bypass services is based on individual group. If you have questions regarding coverage for Gastric Bypass, please contact your benefits administrator or customer service at the number located on the back of your ID card.

I will be traveling out of the country. Will I still be covered?

In the event you are out of the country and require medical service, you will be covered. However, out-of-network fees will be assessed. Be sure to keep records of all expenses incurred. If Medicare is your primary insurance, please contact the customer service telephone number on the back of your ID card for coverage verification.

I am interested in having a specific type of surgery, but wasn’t sure if you covered it. How can I find out if you do?

Answers to specific coverage questions can be found through either your certificate book or Customer Service. If you do not have a certificate book you can request one online through My Health Plan. To contact a Customer Service Representative, please call our Customer Service Department at the number located on the back of your ID card. You can also email Customer Service through the My Health Plan Customer Service email form.
