I would like to appeal a claim, how can I get an appeal form?
You can find the claims appeal form online through My Health Plan, the Member’s section of the Medical Mutual Web site.
You must be logged in to access the Appeal a Claim form. Once logged in, you will be taken directly to that section, or you can find it under the "Forms / Contacting Us" button.
If you have not yet registered for My Health Plan, follow the account activation link on the sign in page. Once you have activated your account, you
will be able to access the form.
If at any time you need help activating your account or using My Health Plan, please call our Help Desk 24 hours a day, seven days a week at
800/218-2205.

I would like to submit a claim. Where can I get a claim form?
You can find claim forms online in the Member’s section of the Medical Mutual Web site.
At this time, it is not necessary to log into My Health Plan to access the claim forms.

I am looking for a recent claim, can I can check that online?
You can find recent claims online through the Member’s section of the Medical Mutual Web site.
You must be logged in to access the My Claims section. Once logged in, you will be taken directly to that section, or you can find it under the “My Health Plan” button.

I am looking for a claim from but do not see it on My Health Plan. How can I get that information?
If your claim is from over 18 months ago, you will have to call our Customer Service department for assistance. You can contact them via phone at the number located on the back of your ID card or email them directly via My Health Plan. Simply go to the Email Customer Service page in My Health Plan.

I would like to ask you a question about a specific claim, can I do that online?
You can email us regarding a specific claim online through My Health Plan, the Member’s section of the Medical Mutual Web site. Simply click the “Email Us Regarding this Claim” button in the claim detail. You can get to the claim detail after logging in by clicking on the My Claims tab and then clicking the claim number you wish to inquire about.
