Cigna Provider Information
Medical Mutual members (excluding ACA and MA) can access the Cigna® PPO network for care provided outside Medical Mutual’s SuperMed® PPO service area. Our SuperMed PPO service area consists of all 88 counties in Ohio and three counties in Kentucky (Boone, Campbell and Kenton).
Important Provider Information
- Eligibility and Benefits Verification: Call toll free at 1-800-362-1279 or log in to the Availity portal and select Medical Mutual as the Payer (Payer ID 29076).
- Cigna Claims Submission: Electronic Claims Payer ID: 62308 / P.O. Box 188061, Chattanooga, TN. 37422-8061
- Cigna will forward claims to Medical Mutual for payment
- Medical Mutual issues the claim payments and remittance advices
- Claims status and Electronic Remittance Advice (ERA) Statements are available through Availity by selecting Medical Mutual as the Payer (Payer ID 29076)

Co-branded Member ID Cards
Our members have co-branded ID cards with both the Medical Mutual and Cigna logos. All claims for services outside our SuperMed PPO service area should be submitted to Cigna.
