Medicare: How to Enroll

Use our simple online application to apply for Medicare, which also let you compare plan options side-by-side so you can find the right plan for you. You can also print a copy of your completed application for your records.
Medicare beneficiaries may also enroll in a MedMutual Advantage® plan through the Centers for Medicare & Medicaid Services (CMS) Medicare Online Enrollment Center at http://www.medicare.gov.

Access your enrollment application below. Please be sure to select a plan that is available in your county. See a breakdown of our plans and counties in Section 1.

Once we receive your enrollment information, you will get a confirmation letter in the mail.
Please note: We do not offer a mailing option for our Medicare Supplement Insurance plans. Please enroll in a Medicare Supplement insurance plan online or by phone.
After You Enroll
Once Medical Mutual starts processing your enrollment form, here is what you can expect based on the plan you selected.
Medicare Advantage Plans with Prescription Drug (MAPD)
We will review your information and confirm with the Centers for Medicare & Medicaid Services (CMS) that you qualify for Medicare.
If everything is in order, we will send you a letter within 10 days to confirm you are a member. Call us at 1-800-982-3117 (TTY 711) if you do not receive your letter before your coverage is scheduled to begin.
Shortly after this, we will send you the following:
- Member ID card
- Welcome Kit
- Mail-order prescription form
- Instructions for requesting directories or a formulary
Medicare Supplement Insurance Plans
After we process and issue your policy, we will send your member ID card and enrollment materials to you. You should receive these within 10 business days of your application’s approval. Your member ID card is similar to other insurance cards you may have had – you will need to bring it with you to use any of the plan’s benefits.
