Our Chronic Condition Management program is designed to help you stay healthy, manage your chronic conditions and maintain your independence. A trained health coach works with you to develop a personalized plan that supplements the care you get from your doctor.
Recover Comfortably
Home Meals Program
Members who have returned home from an inpatient hospital stay are eligible for our Home Meals program. This program delivers two meals a day for seven days directly to your door while you recover at home.
Transportation Services
Members returning home from an inpatient hospital stay are eligible for our non-medical emergency transportation program. Members can receive up to 24 one-way trips to doctor's visits, pharmacies and other medical appointments to help get the care you need.
