If you are enrolled in a Medical Mutual Medicare Advantage plan, you can schedule up to 24 one-way trips to doctor appointments, pharmacies and other medical-related activities. To schedule a trip, call Medical Mutual and we’ll submit your trip request to our transportation provider, SafeRide Health.

How to Schedule a Trip

Contact Medical Mutual Customer Care toll free at 1-800-982-3117 (TTY 711 for hearing impaired) to:

  • Schedule a trip
  • Check on your trip status
  • Cancel a trip
  • Ask questions

Trips are limited to 35 miles, one way. All trips must take place in our Ohio service area, which includes Ashland, Brown, Butler, Clark, Clermont, Columbiana, Cuyahoga, Delaware, Erie, Fairfield, Franklin, Fulton, Geauga, Greene, Hamilton, Hocking, Lake, Licking, Lorain, Lucas, Madison, Mahoning, Medina, Miami, Montgomery, Morrow, Ottawa, Perry, Pickaway, Portage, Sandusky, Stark, Summit, Trumbull, Union, Warren, Wayne and Wood counties.  

Please provide at least 24-hour notice if you need to cancel a trip. For medical emergencies, please call 911. 

Customer Care specialists are available 8 a.m. to 8 p.m., seven days a week, Oct. 1 through March 31 (except Thanksgiving and Christmas), and 8 a.m. to 8 p.m., Monday through Friday from April 1 through Sept. 30 (except holidays).

Your plan covers 24 one-way trips to select locations throughout the year. A trip to a destination counts as one trip and return trip home count as a second trip.  

There is no out-of-pocket cost to use this benefit. 

You can schedule a trip to a variety of medical-related places such as doctor appointments, pharmacies, labs and more. Trips are limited to 35 miles, one way. You, or an authorized representative scheduling the trip on your behalf, must provide the following information:

  • Your name

  • Your date of birth

  • Your Medical Mutual member ID number

  • The pick-up and drop-off location information

  • The date and time of your trip, and your return time (if applicable)

  • If you require any accommodations because of a medical condition 

Yes. To authorize a trusted friend or family member, complete the Protected Health Information release form found at MedMutual.com/PHIForm. To authorize a legal representative, such as a court-appointed guardian or power of attorney, complete the Notification of Legal Representative form found at MedMutual.com/LegalRep

This benefit does not cover trips over 35 miles. 

Please schedule all routine trips at least three business days in advance. If you cannot and have an urgent request, it will be accommodated based on the transportation availability at that time and may not be guaranteed. 

Yes, one family member, friend or personal care attendant over the age of 18 may ride along with you.
