Access Your ID Card
If you need medical care and have not received your ID card, you can download and print a copy of your ID card on My Health Plan, our secure member website. To register for My Health Plan, go to MedMutual.com/member and click Register for an Account. Once you’re registered and logged in, click ID Cards under Benefits & Coverage.

Mental Health and Substance Use Disorder (MH/SUD)
Your plan Employee Assistance Program (EAP) Representative is your confidential resource for personal, family and work-related concerns. Mental Health and Substance Use Disorder services must be provided by Carelon Health, for longer-term care and treatment. The Help-Line number is 1-800-346-7651. For the SuperMed Preferred members, please call the Medical Mutual Customer Care number on your ID card.
Hearing Information
Your hearing benefits are administered by AudioNet, at 1-800-400-2619.
Conifer
Your care management benefits are administered by Conifer, at 888-820-2790. Conifer Health Solutions provides Personal Health Management (PHM) services to eligible UAW-FCA members, spouses and dependents. PHM is a program in which a nurse works with you and members of your healthcare team to help facilitate the high-quality care you need to improve and maintain your health.
Outpatient Physical Therapy
Outpatient physical therapy must be provided by TheraMatrix. You can find a provider or learn more about available services at http://www.TheraMatrix.com or by calling TheraMatrix at 1-888-638-8796.
