Transition of Care Information

If you have already scheduled a medical service for 2021, like a surgery, or you have care that will continue in to 2021, like cancer treatment or maternity services, follow these steps:

  • If the provider is in-network with Medical Mutual, contact your provider. He or she can confirm your Medical Mutual benefits and see if any prior authorization(s) are required.
  • If the provider is not in-network with Medical Mutual, contact Case Western's Human Resources department for a Transition of Care form.*

*Medical Mutual may waive non-network benefits and pay services at the in-network level for up to three months.
