Managing a serious illness, injury or chronic condition can be demanding. Having personalized care, educational tools and practical resources available can help you manage your condition — temporary or lifelong — more effectively and ultimately live a healthier, more fulfilling life.
Medical Mutual members have access to support from our Case Management and Chronic Condition Management teams to help you achieve wellness, stability and independence by taking an active role in improving your health.
- Case Management: Our Nurse Case Managers work to help those dealing with serious illness or injury. Medical Mutual offers personalized advice and support, individualized programs that work with your doctor and more at no out-of-pocket costs for eligible participants.
- Chronic Condition Management: Our healthcare professionals help those living with chronic conditions, like asthma, diabetes and hypertension, with no out-of-pocket costs. Benefits include proactive education to improve understanding of your condition, tips to help self-manage condition and condition-specific supplies.

Self-Care Tip #5: How can I simplify access to my health insurance information?
Managing and organizing your health insurance documents can be difficult. Physical copies can be misplaced, damaged, lost or even stolen. Digital documents — or “going paperless” — keep all of your health plan information in one organized, easy-to-search place. Medical Mutual members can access up to 36 months of EOBs, download documents or review benefits on the device – phone, tablet or computer – you prefer.
Members can log into their My Health Plan portal, hover over the My Profile menu and click on Communications Preferences. If you have questions about communication preferences on your account, contact Customer Care at the phone number listed on your member ID card.
