MedMutual Advantage Wellness Visit
It’s important to check in on your health. Making time for a yearly wellness visit helps establish a baseline, monitor chronic conditions and catch health concerns before they become emergencies.
Medical Mutual believes being proactive about your health deserves a reward. You’ll be eligible to claim a $50 gift card after you complete an annual wellness visit or an in-home health assessment in 2025. Your MedMutual Advantage plan covers both visits with a $0 copay.
Annual Wellness Visit with Your Primary Care Provider (PCP)
Your PCP is your trusted partner on your health journey, so it’s great to check in with them for an annual wellness visit. During this visit your PCP will:
- Take routine measurements like height, weight and blood pressure
- Review your and your family’s medical history
- Review your current medications
- Build a preventive care plan for the year
You can schedule this visit anytime during the calendar year, but the earlier you complete your visit, the sooner you can plan for any other preventive care you may need.

In-home Health Assessment with Signify Health
As a MedMutual Advantage member, you also have the option to schedule an in-home health assessment. During this visit, a licensed healthcare provider from our trusted vendor, Signify Health, will visit your home and:
- Take routine measurements like height, weight and blood pressure
- Discuss your medical history and current conditions
- Complete important tests such as bloodwork, diabetic eye exams, bone density tests and more (if needed)
Consider scheduling an in-home assessment if you’d benefit from completing this visit in the comfort of your home. It also provides one-on-one time to review your health, similar to an annual wellness visit and is designed to complement, not replace, the care you receive from your healthcare team. At the end of the visit, you will receive a summary of your results to discuss with your PCP.
Virtual visits are also available. If you complete a virtual visit with Signify Health, they will send your results in the mail.
Visit Signify Health or call toll free at 1-800-749-4608 (TTY: 711 for hearing impaired) to schedule your in-home visit. You can also schedule virtual visits by phone. Representatives are available 8 a.m. to 6 p.m. Monday through Friday and 8 a.m. to 2 p.m. on Saturday.

After you complete one of these wellness visits, you’ll be eligible to choose a $50 gift card. Access the new MedMutual WELL™ Wellness Portal on My Health Plan.
